Knee replacement recovery- British patient's progress @ 3 months.
Dear Dr.Venkatachalam
Good to hear from you. I’m an impatient person so progress will never seem fast enough for me! However my osteopath tells me she is amazed at my progress and sees me as far ahead of what she has come to expect with all the others she has worked with. I am walking well, and stamina is improving, but still has a long way to go. My wife says that I was walking like an old man in January and early feb., but now I don’t look like one. I am much better at climbing up stairs and am improving at climbing down them, and take most opportunities so to do. I’m up to about 135 degrees of bend, or slightly more, and I think both legs have evened out now.
Today I have been to the Gymn again (Mon, Wed, & Fri), and during the various journeys to and from (and then back to London Bridge for other reasons) have climbed down all the stairs into Canada Water station twice (i.e. 2 x 33 steps), and back up them again, up and down one flight of moving escalator, and then various stairs and steps elsewhere. Down is still more of a problem, and causes pain on the front outside of the knee, below the level of the knee-cap, in both legs but worse in the right. This persists for some time in walking after finishing the steps but goes eventually.
My osteopath says that the muscle you identified ( I still can’t remember the name – VMO – VOM ?)is now very well bulked up and has said to stop using the electro stimulation on it. She is very impressed with the difference that the stimulation has made in conjunction with gymn exercise.

Cartilage surgery can save athlete's joints
There is good news for sports-persons with cartilage injuries in India. At a conference held in Hyderabad, experts including Dr.Venkatachalam of KneeIndia.com discussed the potential of stem cells in bone and joint problems.
One such problem is cartilage injuries of the knee joint. The Knee joint is often injured in sportsmen or even in normal individuals. Ligament and cartilage injuries often occur concomitantly. Although Ligament reconstruction like arthroscopic ACL reconstruction could rectify damaged ligaments, cartilage injuries were left untreated till now.
Reparative cartilage surgery procedures like micro fracture are used by surgeons in low demand patients to repair these lesions by an inferior substitute called fibro cartilage.

Total knee replacement in young patients
Total knee replacement is generally performed in older patients. For the first time in India, a new prosthesis for young patients will be introduced. This prosthesis made of a wonder material called Zirconium, a ceramic coated metal ensures longer survival of the prosthesis after implantation. The introduction of this prosthesis will hopefully encourage more number of young and middle aged Indians and Westerners to undergo knee replacement in India.
Advantages of Zirconium
1) The significant advantage of Zirconium knee implants is that it is considerably harder. On testing it lasts 85 times longer in tests that simulate the stresses inside the actual knee.
Additional advantages are scratch resistance.
2) When it is wet, ceramic can glide smoothly along plastic, making for smoother motion than with metal-on-plastic. As a result, the Oxinium total knee replacement will not generate the quantity of plastic wear debris that cobalt chrome will, and the implants should last considerably longer.
Zirconium is one of the six most biocompatible elements know to man. So, your immune system will more easily tolerate your new implant.
3) Lastly, patients who are allergic to nickel, the metal most often used in the current implants, cannot currently receive total knee replacements. The ceramic implant, however, can be implanted safely in patients with nickel allergies, since it uses a zirconium alloy with no nickel. Implants made of Oxinium have been implanted in larger number of patients in the UK Australia and the US with great success.
Who would benefit most from an Oxinium (zirconium) total knee replacement?
Three types of patient have been identified as the "best" candidates for an Oxinium total knee replacement.
"Younger" patients: Because Oxinium knee implants offer significantly less wear compared to cobalt chrome implants, they are expected to last considerably longer. In the past, orthopedic surgeons have advised patients less than 65 years of age to wait to have knee replacement surgery because the life span of the cobalt chrome knee implants is only 10-15 years. Now, surgeons have a viable option for a younger patient who previously would not be a candidate for a total knee replacement because the Oxinium knee implants should last much longer. Therefore, a patient under the age of 65 who has advanced arthritis of the knee no longer has to suffer for years until he reaches an appropriate age for total knee replacement surgery.
"Active" patients: Patients, who desire to return to an active lifestyle, including sports, dancing, gardening, or other strenuous activities, may also benefit from an Oxinium knee replacement. As a result of their high activity level, this patient population will put more "wear and tear" on their knee replacement, which can cause the implants to wear more quickly. An Oxinium knee is better able to tolerate this high activity level and last longer that the traditional cobalt chrome knee.
Metal allergy patients: Because Oxinium is one of the most biocompatible materials known to man, it is the only choice for patients who exhibit metal allergies. Patients who have needed total knee replacement surgery, but could not tolerate the cobalt chrome implants due to their nickel content, can now have the surgery they need to return to an active lifestyle without the fear of an allergic reaction.
The first patient will be a middle aged Housewife with osteoarthritis.
Whereas normal knee prostheses cost about 80,000 rupees, this new prosthesis may cost about 1. 25 lakhs, but it is comparable to the best in its category. The higher cost will be offset by the longer survivorship of the implant.

1) The significant advantage of Zirconium knee implants is that it is considerably harder. On testing it lasts 85 times longer in tests that simulate the stresses inside the actual knee.
Additional advantages are scratch resistance.
2) When it is wet, ceramic can glide smoothly along plastic, making for smoother motion than with metal-on-plastic. As a result, the Oxinium total knee replacement will not generate the quantity of plastic wear debris that cobalt chrome will, and the implants should last considerably longer.
Zirconium is one of the six most biocompatible elements know to man. So, your immune system will more easily tolerate your new implant.
3) Lastly, patients who are allergic to nickel, the metal most often used in the current implants, cannot currently receive total knee replacements. The ceramic implant, however, can be implanted safely in patients with nickel allergies, since it uses a zirconium alloy with no nickel. Implants made of Oxinium have been implanted in larger number of patients in the UK Australia and the US with great success.
Who would benefit most from an Oxinium (zirconium) total knee replacement?
Three types of patient have been identified as the "best" candidates for an Oxinium total knee replacement.
"Younger" patients: Because Oxinium knee implants offer significantly less wear compared to cobalt chrome implants, they are expected to last considerably longer. In the past, orthopedic surgeons have advised patients less than 65 years of age to wait to have knee replacement surgery because the life span of the cobalt chrome knee implants is only 10-15 years. Now, surgeons have a viable option for a younger patient who previously would not be a candidate for a total knee replacement because the Oxinium knee implants should last much longer. Therefore, a patient under the age of 65 who has advanced arthritis of the knee no longer has to suffer for years until he reaches an appropriate age for total knee replacement surgery.
"Active" patients: Patients, who desire to return to an active lifestyle, including sports, dancing, gardening, or other strenuous activities, may also benefit from an Oxinium knee replacement. As a result of their high activity level, this patient population will put more "wear and tear" on their knee replacement, which can cause the implants to wear more quickly. An Oxinium knee is better able to tolerate this high activity level and last longer that the traditional cobalt chrome knee.
Metal allergy patients: Because Oxinium is one of the most biocompatible materials known to man, it is the only choice for patients who exhibit metal allergies. Patients who have needed total knee replacement surgery, but could not tolerate the cobalt chrome implants due to their nickel content, can now have the surgery they need to return to an active lifestyle without the fear of an allergic reaction.
The first patient will be a middle aged Housewife with osteoarthritis.
Whereas normal knee prostheses cost about 80,000 rupees, this new prosthesis may cost about 1. 25 lakhs, but it is comparable to the best in its category. The higher cost will be offset by the longer survivorship of the implant.

With her left knee bent like ''C'' and the right ike ''K,''
Ejike Eunice (70) of Nigerian orgin had not been able to
focus on her textile business for the last two years
suffering as she was from 'wind swept deformity' due to
osteoarthritis.
The 'obese' grandmother settled in Togu of West Africa
had to take the help of a walking stick for her knees
started bending more and more.
All this until she came in a wheel chair to the
Bharathiraja Speciality Hospital in the city on Novemeber
19. Now, after successfully undergoing Double Minimally
Invasive Bilateral Gender-specific Hi-flex Knee
Replacements, the first African woman to undergo such a
procedure in the hospital, a beaming Ej ike Eunice is all
set to ''walk'' out of the hospital by the end of this
month, without any assistance.
Talking to UNI, Hospital Consultant Or thopaedic Surgeon
A K Venkatachalam, who performed the unique surgery over
two sessions lasting about two and half hours each, said
she was suffer ing from what was termed in medical parlance
as 'wind swept deformity' for the last ten years.
Ejike Eunice's knees started bending progressively over
the years. Though she approached several hospitals in
Africa, they could not help as she was told there was no
standard cure besides citing exorbitant treatment costs.
Dr Venkatachalam said the procedure involved an incision
of only 4-5 inches unlike a conventional procedure where it
could be as long as eight inches.
This approach spared cutting through the thigh muscles
and hence there was less post-operative pain and blood
loss besides early recovery.
''It is more difficult to do and takes slightly more
time than a conventional knee replacement . But the end
results are more gratifying to the patient ,'' he added.
He said the 'gender-specific knee solution' was the only
one of its kind built to suit the female anatomy
accurately. Problems of over size and patellar tracking
have been eliminated from the older prosthesis in this new
knee implant .
The added advantage of the gender-specific knee was it
allowed high flexion or greater knee bending. The user can
sit cross-legged and squat on the floor.
The total cost came to nearly Rs 3.5 lakh whereas in
western countries a similar surgery could cost 6,000 US
dollar per knee, he claimed.
Franca, the patient 's daughter, said the hospital
provided very good treatment and and expressed confidence
that her mother could walk out of the hospital on her own
without the walking stick, and concentrate on her textile
business back home.
The woman, who was under post-operat ive care, was likely to be discharged by the end of this
month.
